Let's talk about how our program will work for you.

Registration Process:

Our registration process includes filling out eligibility paperwork and a short interview with one of our staff members.

Our current application form can be found below.

You'll Need to Know:

  • First, middle, and last names of all household members

  • Age and date of birth for all family members

  • Race and gender demographics for each family member

  • Employment status, place of employment, hours per week, and hourly wages for each family member

  • Monthly income from all other sources (SSI, Retirement, Unemployment, County Assistance, SNAP, etc.)

 

Rules to Follow:

  • All information provided to Community Pathways is truthful to the best of your own knowledge.

  • You agree to inform Community Pathways of any change to be made to your application.

  • You agree to take only what will fill the needs of your family listed on the application.

  • You agree that the items will be for personal use only. Items obtained through our programs will not be resold, bartered, or traded for other goods and services or money. This will lead to a loss of privileges for shopping within Unique Finds and/or The Marketplace. You will follow the directions given for rules on the premises and follow the correct checkout process for all items selected during your visit.

Privacy Notice:

Community Pathways reserves the right to provide data to Steele and/or Dodge County Human Service, the United Way Agencies, MET, Public Health, and other organizations. Any information shared is for research, funding, and demographic tracking purposes only.
